Health management program authorisation form (HMPA)
If you’re claiming benefits for health management programs, you should include this form with your claim. Ask your health care provider to complete the relevant sections of the form.
Medicare claim form
Medicare two-way claim form
Use this form for providers who do not participate in Access Gap Cover, and bill you directly. This form allows Medicare to forward your claim to us for payment of remaining benefits after they have processed your claim.
Medicare rebate form
Use this form if you’d like to receive the Australian Government Rebate on private health insurance as a reduced premium. Lodge your completed form with CBHS.

Recognised provider terms & conditions
These terms and conditions set out the conditions on which CBHS recognises providers for the purpose of paying benefits for Extras services.
HELPER registration form
This form is used to authorise hospitals and their staff to access CBHS’ Hospital Extranet Link for Patient Eligibility Records. The HELPER system gives named individuals online access to private patient eligibility.
Pre-existing condition form
Members may also need to submit this form if they have joined or upgraded their level of Hospital cover in the past 12 months. This medical report allows CBHS’ medical advisors to check whether the member’s condition is a pre-existing condition and determine whether it is covered for the member.
Accident/Injury/Condition Form
The Accident/Injury/Condition Form allows CBHS to access the member’s claim in relation to an accident, injury or condition. You’ll need to outline the nature of the injury or condition and provide other relevant details such as date of occurrence and whether the injury occurred at work.
Pharmaceutical Information Request Form
Use this form to request information from your doctor in regards to consideration of a pharmacy benefit that is not normally payable.
Hospital special consideration forms
On some occasions, CBHS may need to obtain information directly from providers such as hospitals, doctors and other health professionals. These are the forms used for HELPER registration, hospital payments, hospital special consideration, information release authorisation, medical practitioner certificates and accident/injury/condition forms.
